Coinsurance: this is the percentage that you pay after you have paid the deductible and before the plan pays 100% of covered expenses. If the plan pays 100% after deductible or has no percentages for you to pay, enter '0'.
Effective: enter the effective date that would suit you best. Allowed effective dates vary according to the insurance company, e.g., some are only first of the month, some are any day of the month. We will verify with you that your preferred effective date is available for the company that you are applying to.
1st Payment by: enter how you prefer to make the first payment, e.g, credit card, bank draft, or mail a check. Not all companies offer all options. Usually, the first payment with application may be a different method than your subsequent regular payments.
Then Pay by: enter how you prefer to make regular payments thereafter, e.g., automatic credit card payment, mail a check quarterly or six monthly or annually, or a monthly bank draft. Most companies do not allow mailing a check each month because of billing confusion caused by late payments.
